Young Person CHAP Released
The Australian Government Department of Health and Aged Care has released the Young Person Comprehensive Health Assessment Program (CHAP). The CHAP is an evidence-based tool for conducting annual health assessments for people with intellectual disability in Australia. This version of the CHAP is for young people aged 12-18 years.
An annual health assessment is a comprehensive and structured assessment of a patient. There is strong evidence that these assessments are an effective way to identify unmet health needs in people with intellectual disability.
